MAKE THE MOST OF YOUR CARE

A shared understanding.
A stronger relationship.

Clear expectations help us care for you and make appointment time work fairly for everyone.

Read practice policies
How do I book an appointment?

Call 226 270-9077 and speak with a staff member. Appointments cannot be booked by email or SMS. Tell us the reason for your visit so we can arrange the right appointment type. Routine appointments may require a wait of up to four weeks.

How much can we discuss in a visit?

One patient per appointment. A standard 15-minute visit covers one major or two minor concerns. Please prioritize your concerns and request a longer visit when booking if needed; a maximum double appointment may be arranged.

What if I need urgent care?

Call early for limited same-day or next-day acute appointments. Rostered patients can use the group’s after-hours service for urgent concerns. If you cannot access timely care, use an appropriate urgent care service or emergency department. For a life-threatening emergency, call 911.

How do I renew a prescription?

Ask your pharmacy to fax a re-fill request about one to two weeks before you run out. Processing may take up to two weeks. Avoid duplicate requests; ask the pharmacy to wait at least seven days before following up with another fax. Some renewals need an appointment. Call our office if you may run out before your request is processed.

How will I receive my test results?

We contact patients about clinically significant results. Normal or expected results are not routinely discussed. You may call about a week after testing to check. If symptoms persist or worsen, arrange follow-up. Tests ordered by a specialist should first be reviewed with that specialist.

Does applying mean I have been accepted?

No. Completing an intake form does not establish a physician–patient relationship or guarantee acceptance. Spaces are limited. The office will confirm acceptance and next steps. Do not transfer medical records until asked.

Please Apply Only If You Are Seeking a Long-Term Family Physician

Our practice is designed to provide comprehensive, continuous care over many years. Please apply only if you genuinely intend to establish and maintain a long-term relationship with our practice.

We are not accepting applications from individuals who are temporarily seeking a physician, applying to multiple practices with the intention of “doctor shopping,” or planning to transfer again once another option becomes available.

Places in the practice are limited and highly sought after. Accepting a new patient requires a commitment of time and resources and may mean that another applicant remains on the waiting list.

If you are accepted, we ask that you do so with the intention of making this practice your long-term medical home.
